Suzie Costello was introduced by Jack Harkness as his second in command, but Jack was obviously not aware of Suzie’s emotional problems with working for Torchwood. He had no idea that she was the one who had been killing people in order to bring them back to life with the resurrection gauntlet. When Gwen Cooper confronted her, she first shot Jack, and then, she he came back from the dead, shot herself.

Her body was placed in the Torchwood morgue in cold storage, and when, several months later, a series of murders seemed to be connected to her, she was resurrected with the glove. However, it all seemed to be a plan by her to get back to life and kill her father, who seems to have been abusive to her. Jack eventually shot her several times and Ianto had to destroy the gauntlet that was keeping her alive before she was finally dead. Exactly what sent her over the edge in the first place was never fully understood by any of the team. Her own confession that she was tired of seeing the seedy side of the universe seems inadequate.
